As a registered Radiographer (RA68434) your fitness to practise is impaired by reason of misconduct and/or lack of competence in that:

1. On 4 March 2020 you did not provide adequate treatment to Service User 1, in that you:

a. Performed a pelvic x-ray instead of the required hip X-ray;

b. Overexposed Service User 1 to unnecessary radiation;

c. Attempted to delete the image of the pelvis in order to cover-up the error described in 1a.

2. On 10 March 2020 you did not provide adequate treatment to Service User 2, in that you:

a. Performed a pelvic x-ray instead of the required hip x-ray;

b. Overexposed Service User 2 to unnecessary radiation.

3. On 12 March 2020 you did no provide adequate treatment to Service User 3 in that you:

a. Did not check Service User 3’s ID and/or details before the x-ray;

b. Performed an incorrect x-ray because you used an incorrect patient form which was for another patient;

c. Stored Service User 3’s x-ray images in the wrong patient folder;

d. Did not adhere to the ‘pause and stop’ policy.

4. The matters set out in allegation 1, 2 and 3 constitute misconduct and/or lack of competence.

5. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
