Order

ORDER: The Registrar is directed to annotate the Register to show that the registration of Ms Joanna Dunn is subject to the following Interim Conditions of Practice:


1. You must remain under the supervision of a workplace supervisor who is registered as an Operating Department Practitioner with the HCPC, or other appropriate statutory regulator, and you must supply details of your supervisor to the HCPC within 14 days. You must attend upon that supervisor as required and follow their advice and recommendations.

2. You must keep your professional commitments under review and limit your professional practice in accordance with the advice of your occupational health specialist.

3. You must cease practising immediately if you are advised to do so by your occupational health specialist.

4. You must continue to comply with the requirements of your employer with regard to blood testing and/or random breath testing until such time as your employer decides that these are no longer required. You must notify the HCPC in writing within 14 days of your employer’s decision to cease its blood testing and/or random breath testing and you must then make arrangements for the testing of your breath and/or blood for the ingestion of alcohol every three months. You must provide the HCPC with the results of these tests at least 7 days before your Interim Conditions of Practice Order is reviewed.

5. You will be responsible for meeting any and all of the costs associated with complying with these conditions.

6. You must continue to attend regular meetings of Alcoholics Anonymous or any other recognised support group and must provide the HCPC with evidence of your attendance at such meetings, at least 7 days before your Interim Conditions of Practice Order is reviewed.

7. You must abstain absolutely from the consumption of alcohol.

8. You must promptly inform the HCPC if you take up any other employment.

9. You must promptly inform the HCPC of any disciplinary proceedings taken against you.

10. You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake professional work;

B. any agency you are registered with or apply to be registered with (at the time of application);

C. any prospective employer (at the time of application).
