As a registered Physiotherapist (PH117287) your fitness to practise is impaired by
reason of misconduct and/or lack of competence. In that:

1. On 15 November 2019, during a telephone assessment with Patient A:
a. You did not formulate document a clinical hypothesis
b. You did not document an analysis in the records.
c. You did not identify the significance of the patient’s symptoms or the urgency that
was required seek advice from the on-call clinician about Patient A’ s symptoms
and/or make an emergency referral to the Emergency Department for Patient A.
d. you did not recognise and/or record the presence of Cauda Equina Syndrome
and/or take adequate steps to refer Patient A for further treatment.

2. Between April and December 2019, you did not document analysis analysies on the
Subjective Objective Analysis Plan (SOAP) records for the Patients set out in
Schedule A.

3. Between April and December 2019, you did not document aims/goals on the case
records for the Patients set out in Schedule B.

4. Between April and December 2019, you did not complete adequate notes the
outcome measures for the Patients set out in Schedule C in that you did not record
sufficient clinical reasoning.

5. Between April and December 2019, you did not conduct and/or document screening
measures undertaken for serious pathological conditions; for Patients set out in
Schedule D.

6. During and/or subsequent to your assessment of In relation to Patient 5 on 4
November 2019, you did not document the patient’s their history of cancer.

7. The matters set out in allegations 1 - 6 above constitute misconduct and/or lack of
competence.

8. By reason of your misconduct and/or lack of competence your fitness to practise is
impaired.
