Allegation
As a registered Occupational Therapist your fitness to practice is impaired by
reason of misconduct and/or lack of competence and/or a health condition. In
that:
1. Between 1 September 2019 and 14 January 2021 you did not demonstrate
the required skills and knowledge to practice [sic] as a band 5 Occupational
Therapist, in that:
a. You did not complete an informal improvement plan;
b. You did not maintain accurate and complete service user records;
c. You did not demonstrate consistent or accurate clinical reasoning.
2. On or around 30 August 2020 you did not communicate professionally
within written communication with your employer.

5. The matters set out in particular 2 above constitute misconduct.
