As a registered Occupational Therapist your fitness to practice is impaired by reason of misconduct and/or lack of competence and/or a health condition. In that:

1. Between 1 September 2019 and 14 January 2021 you did not demonstrate the required skills and knowledge to practice [sic] as a band 5 Occupational Therapist, in that:

a. You did not complete an informal improvement plan;
b. You did not maintain accurate and complete service user records;
c. You did not demonstrate consistent or accurate clinical reasoning.

2. On or around 30 August 2020 you did not communicate professionally within written communication with your employer.
