Order

 
The Registrar is directed to annotate the Register on an interim  basis to show that, for a period of 18 months, you, Mr Konstantinos Chatzisavvidis,  must comply with the following conditions of practice. 
 
1. You must place yourself and remain under the supervision of a workplace supervisor, registered with the HCPC, and supply details of your supervisor to the HCPC within 14 days of the Operative Date. You must attend upon that supervisor as required and follow their advice and recommendations.
 
2. You must not carry out physiotherapy, unless directly supervised by a HCPC registered Band 6 Physiotherapist or above, or equivalent thereof.
 
3. You must maintain a record of every case you have undertaken, which must be signed by your workplace supervisor, and you must:
 
a. Provide a copy of these records to the HCPC on a monthly basis, the first report to be provided within 14 days of the Operative Date, or confirm that there have been no such cases during that period; and 
b. Make those records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC.
 
4. You must work with your workplace supervisor to formulate a Personal Development Plan designed to address the deficiencies in the following areas of your practice:
a. Professionalism; 
b. Clinical reasoning;
c. Manual handling;
d. Infection control;
e. Appropriate written and verbal communication skills; and 
f. Record keeping.
 
5. Within three months of the Operative Date you must forward a copy of your Personal Development Plan to the HCPC.
 
6. You must meet with your workplace supervisor on a monthly basis to consider your progress towards achieving the aims set out in your Personal Development Plan.
 
7. You must allow your workplace supervisor to provide information to the HCPC about your progress towards achieving the aims set out in your Personal Development Plan.
 
8. You will be responsible for meeting any and all costs associated with complying with these conditions.
 
9. You must promptly inform the HCPC if you commence employment, cease to be employed by an employer or take up any other or further employment.
 
10. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.
 
11. You must inform the following parties that your registration is subject to these conditions:
a. any organisation or person employing or contracting with you to undertake professional work;
b. any agency you are registered with or applied to be registered with (at the time of application); and
c. any prospective employer (at of the time of your application).
 

Notes

This order will be reviewed by the Committee no later than 28 August 2022 or earlier if new evidence which is relevant to the Order becomes available after it was made.
