ORDER: The Registrar is directed to confirm the Interim Conditions of Practise Order imposed on 12 February 2023. These conditions are:

1. You must promptly inform the HCPC if you take up any other or further employment.

2. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

3. You must inform the following parties that your registration is subject to these conditions:

a. any organisation or person employing or contracting with you to undertake professional work;
b. any agency you are registered with or apply to be registered with (at the time of application); and
c. any prospective employer (at the time of your application).
d. any academic establishment where there is the potential for patient contact.

4. You must confine your professional practice to working as a Band 5 Orthoptist and/or an academic Orthoptist role.

5. You must not carry out any patient contact work, as specified in condition 4 above, unless directly supervised.

6. You must maintain a record of every case where you have undertaken patient contact work under supervision. The record must be signed by your supervisor and you must provide a copy of these records to the HCPC 14 days before any review hearing.

This order will be reviewed by the Committee no later than 16 November 2023 or earlier if new evidence which is relevant to the Order becomes available after it was made.
