ORDER: The Registrar is directed to vary the Interim Conditions of Practice Order imposed on 12 February 2023. These conditions are:

1. You must promptly inform the HCPC if you take up any other or further employment.

2. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

3. You must inform the following parties that your registration is subject to these conditions:

a. any organisation or person employing or contracting with you to undertake professional work;

b. any agency you are registered with or apply to be registered with (at the time of application); and

c. any prospective employer (at the time of your application).

d. any academic establishment where there is the potential for patient contact.

4. You must confine your professional practice to working as a Band 5 Orthoptist and/or an academic Orthoptist role.

5. You must place yourself and remain under the indirect supervision of a workplace supervisor. You must attend upon that supervisor as required and follow their advice and recommendations.

6. You must work with your supervisor to formulate a Personal Development Plan addressing the following areas of practice, and provide this to the HCPC within 28 days of the operative date:

a. Appropriateness of clinical testing, investigations and diagnoses;
b. Management plans;
c. Interaction with patients, and their parents, guardians and carers

7. You must forward to the HCPC, 14 days before any review hearing, a report from your supervisor, commenting about your progress towards achieving the aims set out in your personal development plan.

This order will be reviewed by the Committee no later than 26 June 2024 or earlier if new evidence which is relevant to the Order becomes available after it was made.
